The diagnostic fee is its own line, always

Whether you credit the diagnostic fee against the repair or keep it, it belongs on the invoice as a separate line, because it is the only line that pays for the time spent finding the fault. Folding it into the repair makes the repair look expensive and makes waiving it invisible; showing it, and then discounting it if that is your policy, is a stronger position and a clearer record when the customer queries the total months later.

Parts at your price, tax on the parts

The parts line is the marked-up price and nothing else. The markup pays for sourcing, cores, returns and the part you replace under your own warranty next year, and it is not a fee the customer needs to see itemised. Sales tax in most states falls on the parts and not on the labor, so the template applies your rate to the marked-up parts line only and shows the base it was charged on; if your state taxes labor, add it, and the invoice should say which base each tax was charged on.

Shop supplies are a term, not a surprise

A shop supplies charge, usually a small percentage of labor, is legitimate where your posted terms or the estimate said it would be there and your state permits it, and disputed everywhere else. The template defaults to a percentage of labor and lets you set it to zero. Whichever you choose, the invoice names it as shop supplies rather than burying it in the labor rate, because a customer who finds it later trusts nothing else on the page.

Do I charge sales tax on labor?

In most US states, no: services are not taxed and the tax falls on the parts. Some states do tax repair labor, and some tax it only on certain property types. This template taxes only the marked-up parts, which fits the common case, but your state's rule is the one that decides.

Can I add a travel charge as a mobile mechanic?

Yes. Put it in the diagnostic fee line if you charge one call-out figure that covers the drive and the diagnosis, which is how most mobile mechanics price it, or add it to the labor hours at your rate if you charge for the drive as time. Either way the customer should be able to see it, and it should match what you said on the phone.
